A 65-year-old woman was seen in our hospital regarding a foreign-body sensation in the throat over the previous six months. She had a pedunculated tumor 20 mm in diameter in the left palatine tonsil. It was diagnosed as a spindle cell carcinoma by biopsy. There were no confirmed regional and distant metastasis, and the primary tumor was limited to the left palatine tonsil based on the findings of diagnostic imaging methods such as CT and PET-CT. We performed a transoral lateral oropharyngectomy. Histologically, the lesion was diagnosed as a spindle cell carcinoma composed of proliferative spindle cell components and the squamous cell carcinoma and surgical margin was marginal. Therefore, she received adjuvant radiation therapy. The patient is free of relapse 19 months after treatment.
