A rare case of bilateral ovarian metastases following resection of primary ileal cancer is reported, with a review cases of ileojejunal cancer seen the Japanese literature in terms of recurrence patterns. A 54-year-old woman was seen at the hospital because of abdominal pain and vomiting. Plain abdominal X-ray film revealed dilated loops of the small intestine. Patient had a recurrent ileus despite of conservative therapy. Barium enema findings suggested a tumor of the ileum. Laparotomy was performed and the patient was diagnosed as having an ileal cancer without metastasis. Fourteen months after the operation she was admitted again because of ascites. Upon reoperation, bilateral ovarian tumors, were found out and the tumors were resected. Histological and immunohistological studies presented the diagnosis of metastatic ovarian tumors from the ileal cancer. Metastasis to the ovary might occur from lymphogenous or hematogenous route. The ovarian metastasis of jejunoileal cancer is rare as far as we could review. Only four cases have been reported which represented 0.9% of all cases of jejunoileal cancer reported in α recent 5-year period. Ovarian metastasis occurred in younger patients than those without metastasis. Caution to ovarian metastasis should be also employed in patients with jejunoileal cancer during followup period especially in young patients.
