Preaxial prehallux is a rare clinical manifestation among patients with polydactyly. Few cases arising from tarsal bones have been reported. We present a case of a true prehallux occurring in a patient with a bilateral complete cleft lip, palate, and alveolus.

A mobile eye socket is generally reconstructed by inserting an implant into the scleral pocket immediately after bulbar exenteration, or by attaching the extra-ocular muscles to the implanted artificial eyeball immediately after enucleation. However, exposure of the implanted material and other problems can occur. We achieved satisfactory reconstruction of a mobile eye socket by using an autogenous cartilage graft and a pericranial flap in a patient with long-standing anophthalmia due to enucleation. Orbital hypertelorism is defined as an abnormally wide bony interorbital distance. The aims of surgery are both correction of ocular dystopia and cosmetic reconstruction of the nasal crest. Marked improvement of visual function, especially binocular vision, by surgery is not expected. Here we report that surgical treatment unexpectedly resulted in a significant visual improvement for a 13-year-old boy with orbital hypertelorism who also had bilateral cleft lip and palate. Cranium bifidum is a congenital anomaly caused by abnormal development of the cephalic neural tube. We report two cases of cranium bifidum occultum with defects of both the frontal bone and anterior cranial base accompanied by infection and enlargement of frontonasal dermoid cysts. Surgery successfully interrupted the communication between the intracranial space and nasal cavity by inserting a pericranial flap after removal of the dermoid cysts. 藤田保健衛生大学病院口唇口蓋裂センターを受診した粘膜下口蓋裂症例に対し，医療介入効果，治療態度の妥当性評価を目的に後方視的検討を行った。<br>【対象】1992年4月～2008年12月までの粘膜下口蓋裂1次症例50例から，手術，言語獲得に影響したと考えられる合併症を有する14例を除外した36例を対象とした。<br>【方法】軟口蓋正中部で筋層離開のみられるものを粘膜下口蓋裂とし，性別，初診時年齢，主訴，合併症，Calnanの3徴候の有無，軟口蓋の長さと可動性，手術，言語治療について調査，検討した。<br>【結果】男児17例，女児19例。初診時年齢は8日～6歳3か月であった。主訴は，裂などの形態異常21例，言語障害などの機能異常15例であった。合併症は，なし28例，精神発達遅滞11例，22q11.2欠失症候群4例，第1第2鰓弓症候群4例（聴覚障害2例），ロバン・シークエンス 3例（OSAS1例）などであった。筋層離開は36例，口蓋垂裂は28例，骨欠損は22例で認められ，20例でCalnanの3徴候すべてが確認された。軟口蓋長は18例が短口蓋，可動性は22例が良，14例が不良であった。医療介入として手術不要17例，施行19例であった。手術不要群では言語訓練不要5例，施行12例，介入効果として改善10例，やや改善2例，5歳までに言語訓練を開始した11例ではすべて就学期までに治療を終了していた。<br>手術施行群では，口蓋形成術施行18例，口蓋形成術と咽頭弁形成術併施が1例，19例中4例で術後言語訓練不要，15例で訓練を施行した。改善12例，やや改善1例，不変2例，1例はPLP使用中，1例は咽頭弁形成術を追加施行し最終的にやや改善と評価された。手術施行群でも，初回手術後の評価不変2例と，手術が5歳以降であった2例を除き，5歳までに手術を施行した15例では就学期までに治療を終了できていた。<br>【結語】手術施行群初回手術後の評価不変2例を除き，手術不要群，手術施行群とも，5歳までに治療を開始できた症例では就学期までに治療を終了していた。

1992年4月藤田保健衛生大学病院口唇口蓋裂センター設立以来2007年6月までにセンターを受診，登録された口唇裂・口蓋裂患者1130例から資料不備18例を除いた1112例を対象とし統計的調査を行い以下の結果を得た。<br>1）1次症例1073例，2次症例39例，新規登録患者数は1993年～2006年で75.5例/年であった。<br>2）裂型別では片側性口唇（顎）口蓋裂368例（33.1%），片側性口唇（顎）裂279例（25.1%），口蓋裂275例（24.7%），両側性口唇（顎）口蓋裂157例（14.1%），両側性口唇（顎）裂26例（2.3%），その他7例（0.6%）であった。<br>3）1993年～2006年の手術件数は，口唇形成術61.7件/年，1期的口蓋形成術13.1件/年，2期的口蓋形成術での軟口蓋形成術26.5件/年,硬口蓋閉鎖術23.4件/年，顎裂部骨移植術17.8件/年，鼻咽腔閉鎖機能不全に対し2次的に咽頭弁形成術，口蓋再後方移動術を行った口蓋2次手術は3件/年であった。<br>4）言語評価，治療対象として登録された患者数は2007年6月末で，741例であった。
