Auris, nasus, larynx, 52(6) 769-775, Nov 2, 2025OBJECTIVE: To evaluate the safety and feasibility of transaxillary robotic thyroidectomy (TART) in Japan by comparing early cases with video-assisted neck surgery (VANS). METHODS: Single-center retrospective review of 31 consecutive thyroid lobectomies: 16 VANS (Nov 2020-Feb 2025) and 15 TART (Mar 2022-Feb 2025). TART used da Vinci Surgical system (da Vinci) Xi for early cases and predominantly da Vinci SP thereafter. Indications included differentiated cancer ≤4 cm, cN0, unilateral disease without extrathyroidal invasion, or follicular tumors 3-5 cm. Operative time, blood loss, hospital stay, complications, and cosmetic outcomes were evaluated and compared with statistical analysis. RESULTS: All procedures were completed without conversion to open surgery. Blood loss (16.5 g vs 25.8 g) and length of stay (6.3 vs 6.1 days) were comparable. Complications included two transient recurrent laryngeal nerve palsies after VANS and, after TART, one axillary bleed controlled under local anesthesia and one lymphatic leak; overall rates were similar. High cosmetic satisfaction was reported in both cohorts. CONCLUSION: Early experience shows TART is feasible and safe in Japan, achieving perioperative outcomes comparable to VANS with excellent cosmetic results.

喉頭, 25(1) 15-19, Jun, 2013症例は47歳女性で、2年前からスギ花粉飛散の時期に軽度増悪する持続性乾性咳嗽を自覚していたが、特に生活に支障がないため様子をみていた。7ヵ月前から咳が増悪したため前医を受診し、鎮咳薬、抗菌薬、抗ヒスタミン薬、LT拮抗薬、TXA拮抗薬、気管支拡張薬、抗鬱薬、抗不安薬の投薬を受けたが全く有効性がみられず受診した。心因も疑い、精神科に依頼した診察結果は転換性障害と診断され、母親の介護という構成の中で咳をしていることが重要な役割をし、咳を急に止めることは却って問題が大きくなるため、むしろゆっくり経過観察し、変化がなければ抗鬱薬を投与する見解であった。患者から一刻も早く咳を止めてほしいとの希望が強いため、胃食道逆流症とかぜ症候群後遷延性咳嗽に対する併用療法(PPI+麦門冬湯)を2週間行ったが全く変化がなく、マクロライド系抗菌薬併用4週間も効果がなかった。100倍散コデインリン酸塩と麦門冬湯併用を行い、服用4日後より治療抵抗性があった咳嗽が減少した。スギ花粉飛散時期にいったん咳が増悪したが抗ヒスタミン薬併用で軽快し、治療5ヵ月に咳は完治した。

OTOLOGIA FUKUOKA, 53(6Supplement2) S128-S135, 2007Four elderly patients, complaining of an abnormal sensation in the pharynx and/or larynx during swallowing, but without any aspiration of food, received training designed to alleviate their symptoms. The findings of pharyngoesophageal fluoroscopy after training were compared with the pre-training findings. During the training, each patient was instructed to keep the mandible pulled strongly towards the chest, and the trainer manually pulled and stretched the patient's mandible for a while, with the patient's chin immobilized by the trainer's hand. This manipulation allowed the time needed for the passage of contrast material to decrease in 2 cases. Furthermore, following this training, the height of the thyroid cartilage at rest thus became higher than the pre-treatment level in all 4 patients. An improvement in the closure of the pharynx in association with the soft palate was also noted. Although this is a very simple method of training, it is promising as a means of both alleviating a lower-than-normal position of the larynx and suppressing the aspiration of food which is caused by this abnormality.
