    若年者の頻脈性不整脈および心臓突然死の原因の1つである Wolff-Parkinson-White (WPW) 症候群は,心臓内の正常な刺激伝導路とは別に先天的に心房と心室とをつなぐ副伝導路が存在する疾患である.これまでに我々は心房筋・心室筋・副伝導路からなる数理モデルを構築した上で,コンピュータシミュレーションにより,副伝導路の大きさ・心房・心室の導電率・副伝導路の導電率が副伝導路を介した伝導にどのような影響を与えるのかをsource-sink の概念に基づいて明らかにした.しかしながら,イオンチャネル電流と伝導の実際との関連は明らかではない.そこで我々は年齢によるイオンチャネル電流の差異を模擬できる膜電位モデルを構築した上で,心房・心室・副伝導路のイオンチャネル電流が副伝導路を介した伝導にどのような影響を与えるのかについてコンピュータシミュレーションによる検討を行なった.それにより,小児・成人・老年いずれの場合においても,イオンチャネル電流の変化は副伝導路を介した伝導に影響を及ぼさないという結果が得られた.本研究の結果は,副伝導路における伝導の成立およびWPW症候群の顕在化はsource-sink関係が支配的な要因であることを示唆している.

    <sec xml:lang="en"> <title>Background</title> <p xml:lang="en">Detection of the fiber orientation pattern of the myocardium using diffusion tensor magnetic resonance imaging lags ≈12 weeks of gestational age (WGA) behind fetal myocardial remodeling with invasion by the developing coronary vasculature (8 WGA). We aimed to use diffusion tensor magnetic resonance imaging tractography to characterize the evolution of fiber architecture in the developing human heart from the later embryonic period. </sec> <sec xml:lang="en"> <title>Methods and Results</title> <p xml:lang="en">Twenty human specimens (8–24 WGA) from the Kyoto Collection of Human Embryos and Fetuses, including specimens from the embryonic period (Carnegie stages 20–23), were used. Diffusion tensor magnetic resonance imaging data were acquired with a 7T magnetic resonance system. Fractional anisotropy and helix angle were calculated using standard definitions. In all samples, the fibers ran helically in an organized pattern in both the left and right ventricles. A smooth transmural change in helix angle values (from positive to negative) was detected in all 16 directions of the ventricles. This feature was observed in almost all small (Carnegie stage 23) and large samples. A higher fractional anisotropy value was detected at the outer side of the anterior wall and septum at Carnegie stage 20 to 22, which spread around the ventricular wall at Carnegie stage 23 and in the early fetal samples (11–12 WGA). The fractional anisotropy value of the left ventricular walls decreased in samples with ≥13 WGA, which remained low (≈0.09) in larger samples. </sec> <sec xml:lang="en"> <title>Conclusions</title> <p xml:lang="en">From the human late embryonic period (from 8 WGA), the helix angle arrangement of the myocardium is comparable to that of the adult, indicating that the myocardial structure blueprint, organization, and integrity are already formed. </sec>

    National Cerebral and Cardiovascular Center (NCVC) have kept most of autopsy specimens after formalin fixation since founding In order to long save pathological specimens the correct shape after pathology, by making various library and classification, it can be observed the specimens that are stored in a way that suits the purpose to be free, and will be able to contribute as valuable educational materials to physicians and healthcare professionals of posterity In this study, we visualize and reconstruct 3D blood vessel using the digital image of preparation of pulmonary hypertension along with the pathologists of NCVC, and to make the rapid-prototyping of the vessel model.

    We are developing a vector formatted schema system that assists doctors to explain the congenital heart disease to patients. In this system, the users can find schema including keywords such as the name of heart disease and morphological defect of the heart. In addition, the users can collect templates of the heart in different region and combined them to create schema. This system supports doctors to create high-quality schema easily and to explain the congenital heart disease to patients. In this study, we regard the system as filling knowledge gap between health care workers such as doctors, residents and nurses and evaluated the system whether can make them communicate each other efficiency.

    We are developing heart model reconstructing system that extract myocardial region from MR images, visualize heart morphology during one heart-beat and construct heart model for simulating action potential propagation on ventricular muscle such as ventricular fibrillation. However, we have to extract heart surface from all short-axis images manually. Therefore, it took very long time to construct heart model with previous system. In this study, we improved our system to be able to detect heart surface from MR images semi-automatically. Using our new system, we can construct heart model easily compared with previous system.

    We aim to build web services to enable seamless searching and browsing of medical images across the accumulated knowledge, as supporting medical education and more efficient approach in clinical practice for medical students and doctors. Imaging and pathological diagnostic have become essential for medicine. In order to utilize the medical images for the clinical practice, the projects which makes the clinically valuable medical images database is carried out. Therefore, we have developed the application that can search and browse content across. The application was developed by Mashup methods combining web services (Wikipedia, biomedical literature (PubMed), image search and translation). Using this application, the users are able to cross-search from one keyword, and it is possible to next search as a query that is image information and word which obtained by morphological analysis. Then^ this application was evaluated objectively by medical workers. In this application, the stored images and knowledge on web are collaborated, and we can provide services that can get mixed information. Also, we could improve the efficiency of medical education and support precise approach in clinical practice.

    Echocardiography is a common tool in diagnosis of neonatal congenital heart disease. However, it is very difficult to share knowledge of the disease among medical staff from echocardiography. This study aims to develop a 3D cardiovascular modeling system, where medical doctors can interactively edit template cardiovascular models with echocardiographic images. We propose a set of algorithms and interface that facilitate editing of the shape, diameter and topology of 3D cardiovascular models for congenital heart disease. This presentation reports the details of the methods and cardiovascular modeling results using real patient data.

    Analysis of the left ventricular wall motion is important for estimating the mechanical characteristics of the heart. MR Phase-contrast imaging enables us to obtain the three-dimensional velocity data for each pixel. We analyzed the strain in short-axis plane of the left ventricle by using the MR-PC images. In this study, we introduce the new approach based on geometrically-constrained deformation for strain analysis in medical images. We found that the deformation tensor obtained by our method directly leads to mechanical strain tensor of the heart. This approach has low computational load and robust against finite deformation. Our method could provide a clear insight for the complex wall motion of the heart.

    The informed consent in a medical institution is indispensable. However, since a patient or his family has not adequate medical knowledge in general, it often occurs that a patient cannot fully understand explanation from a doctor. Therefore, measures are desired for patients in order to understand enough. For the purpose, we planned to construct an informed consent support system by using IT, especially computer graphics (CG). This time, a trial system for congenital heart diseases especially corrected transposition of the great arteries (corrected TGA) is constructed. Then, we have introduced 3-D animation for showing the operative procedure so that 3-D heart model can be observed from an arbitrary viewpoint. By using this method, it is expected that not only patients but also medical personnel can easily understand the 3-D heart structure and the blood flows along the operative procedure, and the method is useful for the informed consent support system. Moreover, it was evaluated by the questionnaire for doctors, nurses, and students whether the system was effective or not.

    The heart is important internal organs for the human body, and it is known by doing a very efficient systole-diastole movement. It is thought that the twist movement of the heart and the twist arrangement of cardiac muscle fiber influence to an efficient movement of the heart, so various studies are studied.The Phase-Contrast method of MRI can measure the velocity distribution of the arbitrary direction, so we can obtain three-dimensional velocity field by measuring three or more times. The twist movement of the heart can be analyzed directly by using three-dimensional velocity field.It is a problem that the decision of the center of rotation is necessary in case of the method of analysis that uses the rotating angle. In this research, we propose the method of analysis that doesn't need the decision of the center of rotation by using the strain in micro region.First, we calculated the trend of the direction of the maximum principal strain and the normal direction of the maximum shear plane of the numeric model. Next, we compared these directions of measurement data with ones of the numeric model. As a result, three results were obtained 1: In the region with twist in section, the direction of the maximum principal strain shows the vertical direction. 2: In the region with twist between sections, the direction of the normal direction of the maximum shear plane shows the vertical direction. 3: From the time change of the ratio of the region of twist, we got the tendency that the influence of the twist between sections was strong at end systole, and one of the twist in section was strong at early systole and early diastole.

    Human interface is one of the important research topics for effective use of the medical information without limiting it to electronic medical record system. This paper mainly introduces a series of research by our group as shown in the followings. 1) A pen-based interface to systematize free description in the paper clinical record. 2) An effective interface for graphical finding report in cardiac catheterization. 3) A scalable vector graphics interface for congenital heart disease. 4) A system for explaining Senning surgical operation of congenital heart disease with CG. All these are proposals to use visual information effectively for the interface of electronic medical record system mainly composed of textual information. It aims at the achievement of a more comprehensible one.

    It's thought that the twist movement of the heart is important, various studies are studied. The purpose of this research is to analyze the twist movement of the heart with three-dimensional velocity measured by the MR Phase contrast. In this research, time changes of the twist angle, twist rate, the direction of the principal strain and maximum shear strain were analyzed. As a result, in the early systole and diastole, the twist between sections was large, and in the end systole, the twist in a section was large.

    As one of the congenital heart disease, the corrected transposition of the great arteries (corrected TGA) is known. In the clinical field, 2-D illustrations (schemas) are widely used for describing the disease, recording the patients' conditions, understanding the procedure of surgical operations, and so on. We can understand the overviews and the concepts with the 2-D illustrations. However, it is difficult to communicate the 3-D detailed information such as the practical procedure of the surgical operations between the medical professions. So we have developed an interactive system with abstract 3-D graphics for showing the Senning operation's procedure. The user can observe the heart 3-D models with various viewpoints and compare two forms simultaneously. Furthermore, the user can easily understand the 3-D structure and the blood flows along the operation's procedure. And we have interviewed two medical doctors and confirmed that our system brings many benefits for explaining the operations' procedure.

    巨視的なT wave alternans(TWA)は致死性の心室性不整脈の予兆として知られる.一方で,最近,微視的なmicrovolt T wave altemans(MTWA)が心臓突然死につながる心室性不整脈の非侵襲的予知指標として注目されるようになった.TWAの成因としては活動電位持続時間(APD)の回復曲線にもとつくalternansや,不均質性にもとづく興奮波の分裂や回り込みが考えられているが,MTWAの成因については明らかでない.本研究では,コンピュータシミュレーションによりTWAとMTWAを再現し,MTWAの成因とそれが反映する不整脈発生基質について検討した.ヒト心室筋活動電位モデルで1-2次元の正常心筋モデルと,種々のイオンチャネルやgap結合のパラメータを変化させた異常心筋モデルを構成し,計算で描いた心電図からTWAとMTWAを検出した.正常心筋モデルでTWAは再現されたが,MTWAは陽性基準を満たさなかった.異常心筋モデルではMTWAが陽性となる例が存在したが,異常の種類によって,APD回復曲線の急峻化を伴うもの,APD回復曲線の平坦化を伴うもの,興奮伝播異常を伴うものの3つに分類された.MTWA陽性例では,心電図上のみならず膜電位やCa transientの分布マップ上でも,変化が目視では認識できなかった.MTWAは心室性不整脈の発生基質となりうる心筋異常を反映するが,TWAとは種々の点で異なった性質を有しており,MTWAはTWAの延長線上にあるものではないことが推察される.

    Visualization of the ventricle systole and diastole is very important for analyzing the ventricular wall motion. MR Phase-contrast imaging has the advantage that we can directly obtain the voxel's velocity in an arbitrary direction. However, the obtained velocity would consist of various factors, for instance, myocardial contraction and stretch, deformation, and movement in chest. For that reason, it is difficult to visualize the myocardial contraction and stretch from obtained raw data. In this research, we have visualized contraction and stretch in the each part of ventricular wall. We calculated the differential velocity at each target point in transmural and circumferential directions on a short-axis plane. Then we visualized the distribution of myocardial contraction and stretch in the ventricular wall wholly. In addition, we made a mathematical model of left ventricle and compared it with clinical data. It is thought that this leads twisting movement of wholly left ventricle.

    In cardiac catheter test and treatment process, it is much effective to utilize schemata in writing a patient's record. To support this process, we present an intuitive interface system for finding report and treatment planning, such as in coronary angiograms. Using the system, a user can comprehensively draw such as stenosis, blood vessel and bypass and so on. Furthermore the system can also visualize recovering a blood flow after operative treatment. It is suggested that this system can be applied to a component of electric medical record and to an effective explanation tool for patients.

    In this study, we present a new method for constructing of mosaicked images from a series of coronary cineangiograms. The obtained mosaicked images enable us to observe a complete coronary at a glance. In order for medical staff take angiocardiographic images, the staff often have to move a patient's bed in order to catch visualized vessels in the viewing field after injecting a contrast medium. Moreover, those vessels are usually inconsistently contrasted because it takes time for the contrast medium to reach the end of every vessel. In this study, we present a method that reduces such inconvenience and constructs useful coronary images for clinical diagnosis. First, we roughly estimate the heartbeat cycle and the bed movement by using a spatiotemporal image generated from the series of original coronary cineangiograms. Next we group each angiocardiogram frame based on heartbeat phase and estimate the bed movement spatiotemporally. We can then obtain mosaicked images by merging a few frames in the same heartbeat phase with a composite background image. Finally we can easily observe a contrasted, complete coronary in a still viewing field.

    This paper deals with a method of superimposing a coronary arterial tree on a myocardial SPECT image. We can grasp the myocardial function more easily in connection with the shape of the coronary artery. First, a rough shape of the left ventricle(LV)is estimated by using SPECT data. This 3-D LV model is projected on a pair of bi-plane CAG images. By scanning the LV surface projected on CAG, we can obtain 2-D coronary images on a bull's eye map of SPECT. By maximizing a matching degree, registration between CAG and SPECT is performed automatically. Our method has been successfully applied to one artificial data set and several clinical cases.

    We present a method for visualizing a coronary arterial structure on a myocardial perfusion image(SPECT/PET)with Bull's Eye Map.We make a 3-dimensional left ventricular model from SPECT/PET, and project the model on some CAGs according to its imaging parameters.By scanning along the surface of the left ventricular model on CAG, coronary arterial image on Bull's Eye Map is created.
